We would like your first consultation at Los Angeles’s Cancer Center of Southern California to be as beneficial and informative as possible. You probably have many questions about your medical condition, and we would like to answer those questions for you.

Oncology-Gordon-PatientIn preparation for your consultation with us, we ask you to complete a registration form which is accessible online using the link which is provided below. Additionally, we’ve provided a list of items which you are requested to bring with you on the day of your appointment.

Registration Forms

Click on the links below to print and complete each page of the form. There are three forms for you to complete:

1) Medical History
2) Patient Registration Information
3) Medical Record Release

Before you sit down to work on the forms, we suggest that you first collect information about your recent medical care, doctors and insurance. You will be asked specifics about your cancer treatment dates, doctor appointments, contact information for your doctors, hospitalization, surgeries, and prescriptions. In doing this, you will be prepared to answer all questions in one sitting. We provide a glossary of terms that may help.

The registration process will take you approximately 10-15 minutes to complete, and we recommend that you give yourself sufficient time in an uninterrupted environment while you input the information.

Once you’ve completed a page, select the “next” button at the bottom of each page to progress to the next one.

When you’ve completed the last page, select the “Submit” button and your registration form will be received by our scheduling staff.

After you’ve submitted the form online, please call our office if you have not received a follow-up phone call from us within 24 hours to set up your consultation appointment.

What You Need to Bring for Your Consultation

Please bring the following documents with you to your first consultation appointment. The documents can be on a CD if that is more convenient for you, or you can ask your doctors to email or fax the documents to us instead.
